Map: Attacks on Gender Affirming Take care by State

In a coordinated shove led by national anti-LGBTQ+ hate groups, legislators across the country have overridden the recommendations of the American medical establishment and introduced hundreds of bills that target trans person, non-binary and gender-expansive youth’s access to age-appropriate, medically necessary care.

This attack on healthcare care is relentless and constantly shifting. We’ve provided a snapshot of the assault against access to age-appropriate and medically necessary gender-affirming care in the United States.

39.4% of transgender youth inhabit in states that have passed bans on gender-affirming nurture

As reported by The Williams Institute, there are more than 300,000 tall school-aged (ages 13-17) transgender youth in the United States today, many of whom need gender-affirming care.

As of July 2025, 40.1% or 120,400 trans youth aged 13-17 are living in the 27 states that have passed bans on gender-affirming tend . This includes 2,300 youth living in the two states–Arkansas and Montana–where bans are currently on hold or blocked from enforcement through court orders.

  • Despite intense anti-LGBTI attacks in several countries, equality is still advancing across Europe.
  • While the public discourse is becoming more polarised and violent, particularly against trans people, political determination to advance LGBTI rights is paying off. The largest gains on the map are for countries that introduced legal gender recognition using a self-determination model. Over the past 12 months bans on intersex genital mutilation (IGM) are also bringing countries up in the ranking.
  • Spain jumped six places to number four with its introduction of LGR with self determination, alongside a ban on IGM, while Finland entered the foremost ten, again up six places, again with LGR based on self-determination. Greece has also moved up four places with its ban on IGM.
  • Gender individuality and sex characteristics are included in anti-discrimination and/or hate crime legislation, moving Belgium, Iceland and Moldova up the chart alongside Spain.

    Mapping Transgender Equality in the United States

    The Bottom Line

    The lesbian, same-sex attracted, bisexual and transgender (LGBT) community has seen both significant progress and painful setbacks recently. The 2015 Supreme Court ruling in favor of the release to marry was a major step toward legal equality, and there own been many successes since then at the federal, state, and local levels, including increased protections for transgender and gender non-conforming people. However, since the ruling, there have also been more than 200 anti-LGBT bills introduced in 20 states—and a recently leaked executive order draft suggests that the current administration intends to undermine established non-discrimination protections and open the door to a wide range of harms to the LGBT population.

    To help make meaning of the current policy landscape in the states for LGBT people, and transgender people specifically, Mapping Transgender Equality in the United States looks at legal equality for gender diverse people across the state. The gender identity tally is comprised of 25 state laws and policies in the five key categories of Non-Discrimination, LGBT Youth Laws and Policies, Health and Safe

    Snapshot: LGBTQ Equality by State

    The Movement Advancement Project (MAP) tracks over 50 different LGBTQ-related laws and policies.  This route shows the overall policy tallies (as distinct from sexual orientation or gender identity tallies) for each state, the District of Columbia, and the five populated U.S. territories. A state’s policy tally scores the laws and policies within each state that shape LGBTQ people's lives, experiences, and equality. The major categories of laws covered by the policy tally include: Relationship & Parental Recognition, Nondiscrimination, Religious Exemptions, LGBTQ Youth, Health Care, Criminal Justice, and Identity Documents.
